If so, I would describe the scenario as: 1) the rod nut became loose and fell off. 2) the remaining bolt carried 100% of the operating stresses until it finally broke. 3) the rod cap then hinged on the bolt that was missing its nut, bent the bolt, and then flung off into the rotating assembly to see how much more damage it could do... "If" this was the scenario, the rod nut probably wasn't torqued properly during assembly.
